HCG- Online Enrollment Opportunity
Hello Agents,
Hope you, your family and loved ones are healthy and safe during this current crisis.
Due to the COVID-19 pandemic, Health Choice is allowing agents to conduct telephonic sales appointments with beneficiaries and submit online enrollments effective immediately. Please see follow the process listed below.
Online Enrollment Process
A Scope of Appointment (SOA) must be completed before all telephonic sales presentations and meetings.
Sales agents are required to give a compliant sales presentation to beneficiaries or their legal representatives prior to enrolling them into Health Choice Generations.
Once a member decides to enroll in to Health Choice Generations use the OEC link listed below to complete the enrollment.

Upon submitting the online application, a confirmation number will be generated.
Please give the confirmation number to the member. The member will receive a copy of their application in the mail within the next 7 days. They do not need to return it to you or the plan, it’s for their own records.
In order for you to receive credit for the OEC application that has been submitted. The agent needs to send a secure confirmation email to: HCHGenerationsEnrollment@healthchoiceaz.com
Email Subject Line: New Enrollment – A123456789(AHCCCS ID Number)- Doe, John- Effective 06/01/2020
The following three attachments must be included with the confirmation email:
Scope of Appointment (SOA)
Attestation of Eligibility (Currently not printing with application) Use Attachment
Completed HCG OEC Checklist Use Attachment
Applications are also still being accepted via email, fax, and mail.
